BOOK REPORT FORMAT – GRADE 4
 
Answer in complete sentences, starting with a capital letter and ending with an end punctuation mark.
 
Title and author of the book you chose.
 
What is the setting?
 
Who are the main characters?
 
What is the plot of the book?
 
What is the theme of the book? (The lesson learned.)
 
Did you enjoy the book?

How many stars would you give the book from one to five stars?
